Elizabeth Klodas.<\/p>\n<p>Som titlen antyder handler indl\u00e6gget om at et mindretal l\u00e6ger i USA&#8217;s sundhedssystem, heriblandt dr. Klodas, \u00f8nsker at revurdere behandlingen af patienter med forh\u00f8jet kolesterol. Omkring 70 millioner amerikanere lider af forh\u00f8jet kolesteroltal. I stedet for at udskrive kolesterols\u00e6nkende statiner pr. automatik burde l\u00e6ger ogs\u00e5 tage hensyn til andre muligheder for behandling, der ikke n\u00f8dvendigvis omfatter medicin.<\/p>\n<blockquote><p>Did you know that doctors are monitored according to whether they prescribe medications? If I don&#8217;t follow the cholesterol guidelines by prescribing statins, insurers will send letters scolding me. If I don&#8217;t talk to you about the cholesterol-lowering effects of walnuts and oat bran, nobody cares. Physicians even get paid more when a drug is prescribed. A medical encounter that generates a prescription is considered more complex, which qualifies for higher reimbursement. In contrast, if a physician uses some of the very limited time with patients to talk about antioxidants and omega-3 fatty acids, they get nothing more. The guideline mostly instructs providers on which patient to put on which drug and at what dose. Children as young as 10, according to the guidelines, can be started on statin medications such as Lipitor and Crestor. Selvom artiklen m\u00e5ske ikke direkte er en reklame er der dog et link til <em>Step One Foods<\/em> som Dr. Klodas er involveret i.<\/p>\n<p>P\u00e5 <em>Step One Foods<\/em> hjemmeside er der videre et link til fors\u00f8gsresultater, der henvises til i indl\u00e6gget p\u00e5 CNN\u2019s hjemmeside. Fors\u00f8get involverede personer med forh\u00f8jet kolesterol, som var statin-intolerante, og derfor var \u00e5benlyse kandidater til alternativ kolesterolneds\u00e6ttende behandling. Jones et al (se link i bunden af dette indl\u00e6g).<\/figcaption><\/figure>\n<p>F\u00f8rst og fremmest m\u00e5 det noteres, at de fors\u00f8gsdata, som er fremstillet endnu ikke er publiceret efter et kritisk\u00a0 peer-review, det er blot en konference-poster. Basisnievauet for de deltagende personer var p\u00e5 5,65+\/-0,95 mmol\/L totalkolesterol og 3,38+\/-0,83 mmol\/L LDL-kolesterol, s\u00e5 der er ikke tale om fors\u00f8gsdeltagere med t\u00e5rnh\u00f8je niveauer som udgangspunkt. Dern\u00e6st m\u00e5 det konstateres, at der er stor variation p\u00e5 hvorledes fors\u00f8gpersonerne reagerede p\u00e5 behandlingen (varierer fra et fald p\u00e5 37,6% til en stigning p\u00e5 20,5% i LDL-niveau. De tilsvarende tal for total kolesterol er ikke vist). Det resulterede i moderate (om end signifikante) \u00e6ndringer i total kolesterol (forskel p\u00e5 5%) og LDL-kolesterol (forskel p\u00e5 8.8%) ved kostindgrebet. Der s\u00e5s ingen \u00e6ndringer i triglycerider i fors\u00f8get. Den overordnede konklusion, hvis fors\u00f8get kan st\u00e5 til troende, er at kostindgreb kan virke p\u00e5 nogle men ikke alle.<\/p>\n<p>Der er nok ingen tvivl om, at <em>Step One Foods<\/em> og Dr. Klodas har en agenda der nok omfatter noget profit, da de ikke direkte fremh\u00e6ver, hvilke f\u00f8devarer, der fremmer en kolesterols\u00e6nkning, men i stedet sl\u00e5r p\u00e5 at s\u00e6lge produkter som \u00e6sker med \u201dAnytime Sprinkle\u201d, \u201dWhole Oats Pancake Mix\u201d og Strawberry Banana Smoothie Mix\u201d.<\/p>\n<p>Det virkelig interessante er dog, hvad det er for kostr\u00e5d, der kan hj\u00e6lpe i alt fald nogle patienter med at s\u00e6nke kolesterolniveauet. For at komme det n\u00e6rmere m\u00e5 man alts\u00e5 kigge lidt p\u00e5 fors\u00f8gets pr\u00e6misser. Kostindgrebet i f\u00f8rn\u00e6vnte fors\u00f8g omfattede et tilskud af \u2019ready-to-eat\u2019 pakninger (fra Step One Foods) 2 gange om dagen, der giver minimum 5 gram fiber, 1000mg omega-3 fedtsyrer, 1000mg fytosteroler og 1800\u00b5mol antoxidanter pr. portion.
